Customized Number for Jersey

Please indicate 5 jersey numbers in order of most desired to least.  In the event all 5 jersey numbers are already selected (available on a first come first serve basis) the league will attempt to contact the parent listed on the account for an alternate number. If no immediate response is given, the league official will make a choice in their best guess to accommodate the request. At such point, no refund will be given if a response is not received in 48 hours after the request is made or at the time of the final deadline for the season as indicated by the league.
